Reform Physical Therapy & Wellness, LLC is a cash based Physical Therapy practice located within Elemental Pilates and Fitness in Carmel, IN. Reform Physical Therapy & Wellness, LLC was founded by Carrie Webb PT, DPT, ATC and specializes in one on one, personalized care.

Carrie has over 20 years of experience in sports medicine, first as a Certified Athletic Trainer and more recently as a licensed Doctor of Physical Therapy. Carrie also has more than 20 years of experience teaching Pilates and is a Nationally Certified Pilates Teacher (NCPT). Through her Athletic Training, Pilates, and Physical Therapy experience, Carrie has had the opportunity to work with individuals of all athletic abilities and levels, from recreational athletes to Olympians.

As a former collegiate level ballet dancer, Carrie especially enjoys working with dancers of all levels and has had the privilege of working with Dance Kaleidoscope and Butler University’s Dance department in the past.

Reform Physical Therapy & Wellness, LLC is not under contract with any insurance companies, and is therefore considered out of network. This allows both the patient and PT to work together to determine appropriate treatments and a timeline that is not limited or dictated by insurance.
