Do I need a physician’s referral?
Not necessarily. Indiana allows for direct access to physical therapy services without the need for a referral. This is limited to 42 days, so if you are needing care for longer than 42 days, you will need to obtain a referral at that time.
Why should I choose “cash based” PT?
Great question! Cash based PT offers transparent pricing, and high quality care that is not limited by insurance restrictions. Cash based PT also offers one on one personalized care; you will never be in a session with another patient and you will never be transferred to another PT or an assistant. Quality and continuity of care are top priorities in a cash based practice model.
What should I expect in my initial evaluation?
For your initial evaluation, your movement patterns will be assessed, along with your general mobility and strength. It is recommended to wear comfortable clothing so you can move easily. Your personalized goals will be discussed, and a plan of care will be developed to help you achieve your goals. Manual Therapy techniques may be utilized or discussed if necessary to assist with decreasing pain or increasing mobility. Finally you will be given a homework program that is specific to you and your condition. You are an active participant in your success!
How do I schedule?
Scheduling is easy! Just click on the “Book Now” button and schedule yourself! If you are a new patient to Reform Physical Therapy & Wellness, LLC, please select “Initial Evaluation” in the scheduling system.
What is your cancellation policy?
Cancellations that occur more than 24 hours in advance are not charged. Cancellations within 24 hours are charged for the full session amount.
